Ordering an Exam
Referring a patient is easy — simply fax a script or PDI order form to our office. If planning on utilizing the Capitol Dr. office in Milwaukee, please fax to 414-847-1820; if planning on utilizing the Franklin office, please fax 414-529-3102. Same day appointments are welcome — please simply call ahead so we can be sure the patient is prepped if necessary and to confirm the exam time. PDI has two convenient locations.
